CareLink is a 100% volunteer run non-profit medical facility offering limited primary
care to 18 – 65 year old Paulding County residents without health insurance.

Requirements for 1 st Appointment and Visit to CareLink:

 $25 Cash Payment to be applied to initial paperwork and 1 st Dr.’s visit
 Id with current Paulding County address (or ID with current utility bill in your name)
 Most recent year’s tax return filed (2017 or 2018) – all pages
 Most recent 2 months bank statements (checking & savings)– all pages
 If no bank accounts, a simple written statement stating you don’t have a bank account
 3 month’s pay stubs (most recent) – total family income
 If receiving social security or disability – current copy of reward letter
 If receiving child support, copy of court document stating such
 If not working – current statement of such from Department of Labor office
 Offices in Carrolton, Marietta, or Cartersville
 Written statement of support from whom ever (i.e. parent, friend, etc.) is providing room
and board for you if not self- supporting

These documents are required to establish cost of your future Dr.’s appointments and level of
financial assistance that can be provided to you for lab/x-ray tests that may be needed to identify
and treat your health issues.

As we rush towards the end of 2018, CareLink would like to take a moment to reflect upon this past year.  It has been very busy for us.  Following the signing of our definitive agreement and long-term lease with Wellstar in July 2018, we had a very busy three months preparing a long closed Dr.’s office at 625 W. Memorial Dr. Dallas GA to re-open as a Primary Care Medical Facility.  On November 6 2018, CareLink met a four year + aspiration and goal by opening its doors for business, providing a non-profit Primary Care medical facility to the residents of Paulding County that do not have medical insurance.

CareLink could not have accomplished this without the support (verbal, prayerful and monetarily) from the residents and businesses of Paulding County and surrounding counties.  We THANK YOU from the bottom of our hearts.  Every day that we are open and see residents come in that have serious health needs and know in a very small way we are helping them towards a better quality of life, we have renewed gratitude.

We are not done with thinking about and planning for future ways that we can continue to support our fellow residents, but are focused right now on providing primary health care only.  As we look towards 2019, we will continue to seek out those that wish to give either of themselves through volunteering or monetarily so that we can expand our hours and capacity to serve.

But for now we are blessed that we have been embraced, that we have been supported and that we are serving those less fortunate.  Thank you!  As you look towards supporting non-profits, churches, private enterprises both by year end 2018 and into 2019, please consider CareLink of Northwest Georgia.

CareLink Starts Remodel on New Building

CARELINK OF NORTHWEST GEORGIA PARTNERS WITH WELLSTAR TO IMPROVE ACCESS TO CARE AND RESOURCES IN PAULDING COUNTY DALLAS, Ga. (July 18, 2018) 

CareLink of Northwest Georgia, Inc., the county’s first community health and resource center for the under-insured and uninsured, is partnering with WellStar Health System to help provide accessible and affordable healthcare and services to eligible Paulding County residents.

Everyday, people in Paulding County make difficult decisions about whether to seek timely medical care or go without because of financial limitations. They can now get their primary healthcare needs met at the CareLink Medical Facility. The partnership with WellStar is helping remove barriers to primary health care by improving healthcare access for uninsured and under-insured Paulding County residents, ages 18 to 64. CareLink will deliver healthcare and support services to those in need through the partnership. Services in planned order of implementation are:

  • Primary healthcare
  • Dental care
  • Behavioral and Wellness health
  • Community Resource Center

“At CareLink, we have a very clear mission—to deliver healthcare and resources to those in need,” said Guy Scull, president of CareLink, a non-profit organization. “Paulding County stakeholders and residents have worked tirelessly for over three years to bring this health and resource center to their community. With WellStar’s generosity, we are poised to make a significant impact in Paulding County as we improve healthcare access and address social determinants of health through our expansive network of community resources and partners.” CareLink plans to open their medical facility by early October 2018, following some upgrades and renovations at the facility.
